Having worked as a rural medical officer in South Africa, I would 
STRONGLY RECOMMEND the following books:

Primary Surgery, Vols. 1 and 2 (King ed.)
Obstetrics in Peripheral Hospitals (Larsen)
Primary Child Health (King ed.)
Training Health Care Workers (Abat and McMahon)
Lecture Notes on Tropical Medicine (Bell ed.)

In addition, I would like to draw attention to a recent manual entitled 
"Anaesthesia for the Rural Doctor" which you may like to consider.
