RFI: Elusive article

I am a student at the Nuffield Institute for Health, Leeds University 
studying for a Master of Public Health (international). I am desper-
ately seeking the following article:

Collins S.  Ignoring the host: the impact of recent refugee crisis on 
health infrastructure in Ngara District, Tanzania. Amsterdam, Medicines 
Sans Frontieres 1996:1-89.

I have already contacted MSF who have no record of this article. My 
only hope is that someone, somewhere has come across the article and 
knows where I can find it. It is vital that I find this article for my 
dissertation "The disparity of community health care provision between 
refugees and host communities".

If anyone has any information regarding this article or any article re-
garding the impact of refugees on host communities, could you please
