RFI: Quinine Resistance - Ifakara, Tanzania

We are Medical doctors working in the paediatric department of St
Francis, a District designated hospital located in a rural malaria
hyperendemic region of Tanzania. The community has low income and
limited resources. Due to high rates of chloroquine resistance, we
are now changing the firstline antimalarial to Fansidar, however cur-
rently both drugs are used as firstline antimalarial drugs. The sec-
ond and last drug of choice is quinine, we are now facing frustrating
resistance to this drug. In our ward we have been coming across qui-
nine resistant cases several times most resulting into mortalities,
now we have three neonatal patients with congenital malaria, they
have been under quinine treatment for thirteen days however their
parasitaemia levels have never been affected by the drug. We are
seeking for advice, probably a long-term solution. We will be grate-
ful if you can help.
