Hello:

Of interest is your requirements to participate:

* Established medical researchers or clinicians in their respective 
  countries, who 

* Work in developing countries, this being arbitrarily defined as 
  those countries ranking 65th or lower in the Human Development Index 
  1999. (Appendix I)

* Are prepared to enter into a research affiliation with the Univer-
  sity of Toronto Office of International Surgery

* Have or can get regular access to the Internet

* Agree to do all their electronic health resource work using the 
  designated proxy server account

* Agree to participate in surveys to assess their use of and satis-
  faction with the proxy server access supplied.

* Sign a consent form acknowledging that their use of the Internet 
  will be monitored while they are connected through the University of 
  Toronto proxy server.

* Agree not to sell the information they obtain or redistribute it 
  for financial gain nor allow others to use the service provided for 
  financial gain

* Acknowledge that the University of Toronto Office of International 
  Surgery retains the right to discontinue their access to the proxy 
  server at any time without any form of compensation.
