RFI: Malnutrition follow-up

I am a clinical researcher working in the area of tropical child 
health. I am interested in following up children discharged from the 
wards after treatment for malnutrition. I am looking for information 
about any tools that have been developed for post-discharge follow-
up/management of PCM. I would be glad if anybody interested in malnu-
trition would let me know.
