Dear Peter and David,

I guess both your arguments without any particular research on my side 
are correct. Over the years I have heard of many cases where people 
have died and we have come to a conclusion that it was medical negli-
gence which is either a result of lack of equipment or motivation. Un-
fortunately for long none has raised an alarm. Even the researchers 
have not thought of attempting to attribute cause of death from this 
background. Maybe it is a challenge for those who are close to clinical 
practice to think through this.
