Sponsorship

JHPIEGO does not provide financial assistance to applicants, and
will not mediate between applicants and funding organizations.
JHPIEGO is also unable to provide addresses or other contact in-
formation for potential funding organizations.

Applicants who need sponsorship should consult the local Minis-
try of Education or Higher Education, Health, or Community Ser-
vices. Alternatively, applicants may seek such sponsorship
through the local offices of international organizations such as
the World Bank, the World Health Organization (WHO), the United
Nations Population Fund (UNFPA), the United Nations Children's
Fund (UNICEF), the United States Agency for International Devel-
opment (USAID) and USAID supported agencies, or private founda-
tions.

Applicants seeking funding for this program are advised to con-
tact potential sponsors as soon as possible. It is recommended
that participants secure sponsorship early to ensure that your
space at the workshop is not forfeited.

Documentation of Sponsorship

Please have the sponsoring agency complete the attached sponsor-
ship form, which should be submitted along with other applica-
tion requirements.

Insurance

United States health insurance is mandatory for international
participants. The sponsoring agency is responsible to provide
the participant with short-term health and accident coverage. A
copy of the certificate attesting to health and accident insur-
ance coverage is to be sent by fax to JHPIEGO by 1 September
2004.

Visas for International Participants

International participants should request assistance from their
sponsoring agency for obtaining the requisite visa to travel to
the US, if needed. Upon registration of the participant for the
workshop, JHPIEGO shall provide notice in writing to both the
participant and the sponsoring agency. This documentation may
be used to support the visa application to the US Embassy in the
participant's country

It should be noted that due to increased security checks at U.S.
Consulates overseas, it might take several months to secure an
appointment and the appropriate visa approval to enter the
United States. Please plan accordingly and make your application
to the U.S. consulate, allowing sufficient time, according to
the requirements of the Consulate in your country.

Workshop Location

The workshop will be held at JHPIEGO, 1615 Thames Street, Balti-
more, Maryland, USA.

Accommodations in Baltimore

Participants are advised to obtain accommodation at a hotel that
is in close proximity (walking distance) to JHPIEGO. The hotels
listed below have each agreed to hold blocks of 20 rooms for
reservation by participants at this course

Admiral Fell Inn, 888 South Broadway, Baltimore 21231, (Tel No.
+1-410-522-7380): US$ 139 per night, plus taxes

Henderson's Wharf, 1000 Fell Street, Baltimore 21231, (Tel No.
+1-410-522-7777 or toll free +1-800-359-6396): US$ 169 per night
plus taxes (includes daily continental breakfast, internet con-
nection, fax machine and printer)

The blocks of 20 rooms each will be held, and the rates noted
above guaranteed by the hotels, until August 20, 2004, and will
be provided when the reservation codes as shown below are cited:
Admiral Fell Inn: #59007
Henderson's Wharf: JHPIEGO September course

Alternatively, reservations may be made via the Internet at
other Baltimore hotels, using websites such as Expedia or Ho-
tels.com. Please note that accommodations obtained at Baltimore
hotels other than the two noted above will likely require addi-
tional daily expense for transportation to and from the workshop
location. Public transportation routes pass five blocks away
from JHPIEGO offices, thus taxis are the more convenient, but
more expensive, mode of transportation. This additional expense
should be factored in when accommodation is obtained elsewhere.

Accommodation reservations are the responsibility of the par-
ticipants and their sponsoring agencies.

Cancellation/Refund Policy

In the event of a cancellation by the participant, the refund of
tuition policy is as follows:
Cancellation 30 days or more prior to Course - 100% refund
Cancellation between 15 and 30 days prior - 50 % refund
Cancellation less than 15 days prior - No refund

Notice of cancellation must be received in writing by mail, fax
or email. JHPIEGO will issue a confirmation notice to the spon-
sor upon receipt of written notification of cancellation.

JHPIEGO also reserves the right to cancel the workshop due to
low enrollment, in which event all paid fees will be refunded to
the participants or sponsoring agency.
