Help Needed with Questionnaire Development

Hi,

I am working in a demographic research centre of a developing
country. We are planning to carry out a survey on ageing,
health, social security and family relationship.

The major variables of interest are:
1. perception of the elderly
2. socioeconomic condition of the elderly
3. care provider attitude
4. Socio-cultural factors
5. family relationship
6. role and status of the elderly
7. income level
8. gender and the old

Can someone help me with design of questionnaire for this study?
