Intensified Support Assistance Collaboration for HIV ? Indonesia
(ISAC ? HIV)

Proposed Terms of Reference

Short Term Professional ? HIV Care

The National AIDS Program Indonesia has made major efforts in
acceleration of Indonesia HIV/AIDS Comprehensive Care expansion.
A Five Year AIDS strategic plan for 2003-2007 is in place and
has been widely distributed. The MoH will be expanding compre-
hensive HIV prevention and care to 17 provinces of Indonesia un-
der its commitment to the Sentani Declaration. Donor support
from GFATM 1 and round 4, USAID ? FHI ? ASA, AusAID ? IHPCP have
been secured to support this expansion. A Central management
structure has been established to administer funds in an effi-
cient manner. Provincial Management structures are being formed,
through appointment of Provincial Implementation Units (PIU), to
work closely with the Provincial authorities and partners and
manage the GFATM supported activities.

Within the 4th Round GFATM proposal, the Ministry of Health has
determined three major goals. The first focuses on providing ap-
propriate care, support, and treatment for people living with
HIV/AIDS (PLWHA), an area that has remained undeveloped because
of extreme limitation on resources. With technical support from
WHO?s 3 X 5 Initiative and this proposed assistance from the
Global Fund, sufficient resources and expertise will become
available to provide the necessary care, support, and treatment
for HIV+ people throughout the 17 target provinces. The second
goal focuses on prevention of HIV infection among the three tar-
get populations where the epidemic is most widespread; commer-
cial sex workers, injecting drug users, and transvestites. The
third goal focuses on prevention of infection among vulnerable
men through workplace programs in targeted industries. By pro-
moting safe sex, condom use, and health seeking behaviours among
this important ?bridging? population, the spread of the disease
into the general population should be avoided. All of these
three goals are synergistic, and will help to significantly ex-
pand current interventions and augment the national response to
HIV/AIDS.

For the following areas detailed work plans and achievement in-
dicators have been developed and will be addressed during the
period of 2004 - 2009: Testing and Counselling, including VCT,
Treatment and Prophylaxis for Opportunist Infections, ARV Treat-
ment and Monitoring, Palliative Care, Health System Strengthen-
ing, Procurement and Supply Management Capacity Building, Pro-
gram Management Support.

WHO Indonesia is now looking for three experts to support the
teams in Jakarta and Papua in providing technical support.

1) Short Term Professional for HIV Care, ISAC ? HIV will be
based in Papua ? WHO Office and working closely with the MO 3 by
5, WHO and be expected to:
o Establish and maintained close contacts with various govern-
ment counterparts in the MOH, the Provincial Programme Coordina-
tor and other relevant agencies with respect to coordination of
the ISAC ? HIV programme.

o Achievement of the ISAC ? HIV in commitment to the GFATM grant
and the WHO 3 by 5, is implemented properly through quality of
care.

o Provide technical assistance on the development of project ac-
tivities in accordance with the provision of the ISAC ? HIV
technical assistance needs.

o Advise in the development of provincial POA to set up systems
and procedures necessary to ensure quality and sustainable HIV
care services.

o Advise in the development of an integrated comprehensive com-
munity social mobilization and advocacy strategy and POA to in-
crease the demand from the community and to improve community
awareness for Testing and Counselling, ART services ( in coordi-
nation with WHO and partner experts).

o Strengthen Key Services for HIV treatment and care scale up,
including development of operational guidelines and training;
outreach programmes, treatment availability for ART and case
management and counselling; Laboratory monitoring of ART and
drug resistance.

o Advise, develop user friendly training tools, plan and accel-
erate training efforts in the provinces/Districts in HIV & ART
services.

o Ensure standardize data collection methods (treatment, cover-
age, nation wide regular collection for project level data on
same indicators) together with country partners in order to com-
plement/ contribute HIV data (coverage, quality, inputs and out-
puts etc ) to existing efforts to strengthen M&E plans and/or
relevant health information systems towards a standardized na-
tional data.

o Advise and assist methods for collecting information on the
HIV programme, including surveys and operational researches.

o Closely monitor the implementation status and carry out regu-
lar follow ups for timely implementation of HIV care related ac-
tivities and reporting to WHO and PMU ? GF AIDS/MoH.

o Facilitate good coordination among sectors within the MOH pro-
grams such as the medical care for health system strengthening,
TB program, community health program, laboratory quality
strengthening programs, and with universities, academic bodies,
INGOs/NGOs for public ? private partnership activities.

o In discussion with the Medical Officer for STIs & HIV/AIDS,
the Medical Officer for 3 by 5, Indonesia and the PMU - GF AIDS,
advise and assist in the development of the activity proposals
for implementation at provincial level.

o Assist with other areas as may be assign by the WHO and re-
quest for support by the MOH.

Duration of Assignment: 11 months

Qualifications Required:
Medical degree or advanced postgraduate training or degree in
epidemiology/public health/control of communicable diseases with
extensive international experience, preferably in HIV Treatment
& Care.

Skills and Competencies
o Considerable experience in the implementation of national HIV
& AIDS prevention and control programmes at the international
level.

o Extensive management and technical knowledge and technical ex-
pertise in public health disease control, care, support & treat-
ment of HIV/AIDS especially with ART.

o Experience and skill in data analysis, proposal development
and project management.

o Ability to work effectively with colleagues at the national
and international levels.

o Broad-based knowledge of WHO's policies and programmes, and
those of the UN system.

o Ability to perform independently under minimum supervision.

o Well-developed communications skills, both oral and written.

Desirable:
Specific knowledge of Antiretroviral and Clinical Management of
HIV patients.

Languages:
Excellent command of written and spoken English.


2) Short Term Professional for STI, ISAC ? HIV will be based in
Jakarta ? WHO Country Office and will be expected to:
o Establish and maintained close contacts with various govern-
ment counterparts in the MOH and other relevant agencies with
respect to coordination of the ISAC ? HIV programme

o Provided technical assistance on the development of project
activities in accordance with the provision of the ISAC ? HIV
technical assistance needs.

o Advise in the development of POA to set up systems and proce-
dures necessary to ensure quality and sustainable STI services
for populations at risk such as Sex Workers, IDUs and youths.

o Strengthen Key Services for STI treatment and care scale up,
including development of operational guidelines and training;
outreach programmes, treatment availability for STIs and case
management and counselling; and providing user friendly compre-
hensive STI services for at risk population groups such as
youths, laboratory monitoring of STIs and drug resistance.

o Advise, develop user friendly training tools, plan and accel-
erate training efforts in the provinces/Districts in STI ser-
vices.

o Advise and develop Standardize data collection methods (treat-
ment, coverage, nation wide regular collection for project level
data on same indicators) together with country partners in order
to complement/ contribute STIs data (coverage, quality, inputs
and outputs etc )to existing efforts to strengthen M&E plans
and/or relevant health information systems towards a standard-
ized national data.

o Advise and assist methods for collecting information on the
STI programme, including surveys and operational researches.

o Closely monitor the implementation status and carried out

donesia and the PMU ? GF AIDS advise and assist in the develop-
ment of the activity proposals for implementation

Duration of Assignment: 11 months

3) Short Term Consultant (Laboratory Expert) - ISAC ? HIV will
allow two weeks of time x 4 visits a year to the WHO Office in
Jakarta and be expected to: (In consultation with WRO and na-
tional authorities)

1st visit:
o assess the status of laboratory support available in the coun-
try for HIV diagnosis and monitoring of ART, identify the major
gaps and develop a detailed work-plan for enhancing the effec-
tiveness of lab support to HIV programme including training
needs, supplies and equipment.

2nd visit:
o Provide technical support for implementation of the work plan
developed in first visit including training and installation of
equipment.

3rd visit:
o Provide technical support for implementation of the work plan
developed in first visit including training and installation of
equipment, and
o Review the progress made, identify need for any midcourse cor-
rections.

4th visit
o Provide technical support for implementation of the work plan
developed in first visit including training and installation of
equipment and

o Review the progress made, identify need for any midcourse cor-
rections.

o Plan for the activities for next year.

In all the visits following additional ToR:
o Regular supervision, monitoring and reporting of the ISAC ?
HIV.

o Assist with other areas as may be assign by the WHO and re-
quest for support by the MOH.

Duration of Assignment: Two weeks x 4 visits per year

Qualifications Required:

Postgraduate qualification (MD/MSC or PhD) in microbiol-
ogy/laboratory sciences or laboratory degree and considerable
experience.

Establishment of lab services for HIV diagnosis and ART monitor-
ing at national level.

Experience in imparting training/teaching laboratory staff in
techniques for HIV diagnosis and ART monitoring.

Experience in undertaking monitoring of lab services preferably
those dealing with HIV

Languages:
Excellent command of written and spoken English
