Thank you for the summary - "magic bullet approach".
"An alternative strategy to reduce maternal mortality" by Anthony Costello, Kishwar Azad and Sarah Barnett is recommended; in the comments section is the silver lining. They have made a compelling argument in favour of alternative context-specific services and community-based approaches in addition to the health centre-based intrapartum care. In my opinion (coming from a high maternal mortality area in Jharkhand and Orissa in India) PHM should subscribe to such a view.
