I shall be happy if anybody refers me any website where I can get baseline and evaluation questionnaires on community cancer awareness (about avoiding the habits resulted in cancer, early cancer identifications and identification of cancer patients etc.) or community KAP baseline and follow-up structured/semi structured questionnaires on the disease cancer).
