Dear Dr Burgess & Asghar

I am a simple Italian MD who worked in Mozambique for some years as consultant of the Médico Chefe Provincial (the second regional authority, in Sofala) and I strongly agree with you.

The vertical programs dismantle the slowly and fatiguing achieved results in building a weak NHS, but is also true that resources delivered at central/provincial level (as a budget support too) take thousands directions, included the pockets of health workers, usually underpaid and exposed to open such a pockets (this is not the main stream anyway!).

The way to arginate such a dispersion is most probably to distribute funds/resources at district level (always as a budget support, as we tried = these way is tuning with the expected empowerment, promoting self reliance, accountability and so on), where the above mentioned pockets are still empty but narrower (we can accept some money "shifting" in front of coping the daily hard life with low salaries) and everybody has the funding outcomes under their own eyes (included those of the customers - sick people's eyes), meaning an easier way in controlling the funds allocation.
