Won?t be able to say much since I am not seeing your wife here. But only to let you know that IUCD use in itself has a risk of pelvic inflammatory disease (PID) as the foreign body provides a surface for micro organisms grow. Not to mention the fact that she used it for 6 years. The symptoms are similar to STI in women.

You need the services of a gynaecologist.
